What to Actually Expect in Your First Therapy Session
Taking the leap into therapy can feel a bit like showing up to a party where you don’t know the dress code:
Do I spill my life story in the first five minutes? What if I accidentally cry into my coffee? Should I make a slideshow of my life so far?
At Nuance, we get it. Starting therapy is new territory, and that can spark plenty of anxiety. The good news? Your first session isn’t an interrogation—it’s a conversation to help us figure out how to best work together and get the ball rolling.
Here’s what our first 50–60 minutes together will actually be like:
1: The Necessary Housekeeping
(Getting the Boring Stuff Out of the Way)
Before we dive into the deep stuff, we start with the operational ground rules. We’ll briefly walk through informed consent and review administrative policies so there are zero surprises down the road. This covers:
Confidentiality: What stays between us (which is almost everything, with a few legally mandated exceptions);
Payment & Billing: How fees, insurance, and payment work, and what to expect;
Rescheduling & Cancellations: What to do when unexpected schedule conflicts come up;
Telehealth Policies: What we need to make sure sessions are secure and running smoothly.
We keep this part quick, clear, and painless so we can focus on you.
2: Unpacking the "Why Now?"

Next, we pivot to what brought you through the door (or onto the screen). You don't need to have anything prepared. We’ll simply open up the conversation to what’s currently feeling heavy, frustrating, or stuck in your life right now.
Whether you're navigating anxiety, burn-out, ADHD, or relationship dynamics, we want to hear about it.
3: Context & History
(Connecting the Dots)
To really understand what you’re experiencing today, we need a little backstory. We’ll zoom out to build a helpful snapshot of your history: relevant past experiences, family dynamics, past therapy journeys, or lifestyle factors that contribute to what’s going on now.
Think of it as gathering the pieces of a puzzle, so we can see the full picture together.
4: Goals, Game Plans, and Next Steps
As we get to understand what’s going on, we’ll transition into what our work together will look like. What would real progress look like for you? We’ll discuss treatment focus and how to build & work towards your goals.
Towards the end of the session, we’ll touch base about how the conversation felt for you so we can adjust as needed. Finally, we’ll wrap up by confirming a recurring meeting time that fits into your life.
